Question:
I’ve been plagued with sinus problems and headaches for a long time. My doctor has had me on Claritin for well over a year and it doesn’t appear to be helping. I’ve read a lot about irrigation and am wondering if this will help my sinus to drain. I’ve used every otc remedy available and they really do no good. Any suggestions will be most welcome.

Is your doctor an otolaryngologist or a G.P.? Antihistamines can have a drying effect on the sinuses that causes moer problems than it solves. Also, what do you mean by "sinus problems." That is a conclusion rather than a description of symptoms.

Have you ever told your doctor that the Claritin was not working, and that you hope he/she would have some other modes of treatment to try? Larry

If you EVER have a problem that a doctor hasn’t helped in over a year, you need to get a second opinion. In this case, you could go either of two ways: If you strongly suspect you have allergies, then you should see an allergist. Otherwise, you should see an otolaryngologist (ENT). Because in either case, a family physician won’t be of much help. Family (primary-care) physicians simply don’t have the training to do the kinds of tests and examinations that these specialists can do. Time for me to get on my soapbox: Over the last 30 years, there has been a HUGE advance in diagnostic medicine: CT scans, MRI scans, endoscopy, etc. But little or none of this has filtered down to the primary-care physician. The examination the primary-care physician gives today, differs little from the way family physicians examined patients 30 years ago: Look in the ears (otoscope), listen to chest (stethoscope), take patient history. That’s it. And that’s nearly useless for dealing with sinus problems. — Steven D. Hi here’s my reply to your 2 posts about the faq and your desire to get a good nights sleep. The last faq update was on 3/4/01. There are lots of ways to retrieve it. One way is to use Google Groups search at http://groups.google.com/ then enter "sinusitis faq" (be sure to spell it correctly in the search box, then click on "sort by date" and you’ll get it by going back a page or so. Couple thots about a good nights sleep. I would obtain Dr Grossan’s sinus tip that you use with a water pik. You’ll find info on this at his website at www.ent-consult.com. Use it to clear out your nose/sinus area in the evening. I also use Dr Grossan’s Breathe-Ease and Clear-Ease. All good stuff. In the interim, try a hot steamy shower in the evening, hot packs on the face, squirt saline water into your nose (ayr, ocean etc), try eating hot spicy food. The objective is to get your nasal/sinus area to drain. I get relief taking Maximum Strength Tylenol Allergy Sinus at bedtime. It doesn’t bother my sleep. I think there’s even a PM option for taking at night. We’re all a little different with medicines. For me, the Tylenol does not help with pain but the antihistamine & decongestant works good. For my pain, only Excedrin works. Here’s my broken record message. You gotta do a lot of trial & error methods and in there somewhere will come some help. Keep plugging away. And follow the directions you get with all this stuff. A lot of us have made mistakes and caused more problems. Good luck and keep us informed with your progress……. Also, try elevating your head when sleeping. Some people put blocks under their bed legs at the headboard to raise the whole bed. Adding pillows or rolled up blankets under your head can work too. This takes some time to get used to sleeping with your head raised but you can change your habits. This helps the sinuses to drain while asleep. Hello (I tawt I taw a) Puddy – tat…. I have found 2 things that have worked wonders for me in controlling the (never ending) sinus headaches that I have experienced: The water-pik with the Grossan irrigator tip and Flonase spray. I irrigate about 4 times a day, and I give one spray of Flonase in each nostril, first thing in the morning — then repeat spray 15 minutes later. ….It is important not to over-do the Flonase spray, as spreading it over the day or exceeding a twice a day shot , actually contributes to the congestion. Ie: don’t use it as an anti-histamine/decongestant sprayer. Additionally, about 15 minutes after I irrigate, I lay on the bed, hang my head over the edge, and by rotating my head every few minutes from one side to the other, I allow the water to drain out. ….It also helps considerably to hold the nose a few times, while in this position and swallow (this creates a vacuum, which helps pull material out of the sinuses) …. Further… I never blow my nose as if I’m trying to force my brain out through my nose. I blow very gently, as Dr. Grossan suggests, with the mouth open. I recently had a very bad cold a few weeks ago, and this method got me through the subsequent sinusitis aggravation without even having to resort to another round of antibiotics. Dear Jon, It isn’t a good idea to irrigate with the Water Pik 4 x a day unless you are trying to prevent a common cold after bad exposure to others with colds. If you do 4x a day you remove Lysozyme and other products that the body makes to combat infection. Lysozyme and other surface products kill the bacteria in a natural "good" manner. Therefor twice a day should be the desireable frequency if you need it. For the common cold to enter your body, it requires a natural product called ICAM 1 in order to enter. If you irrigate frequenly you remove the ICAM 1 and so there is no entry way for the cold virus. This is a mighty complicated subject…in general, migraine and allergies have relatively little to do with each other, and food testing, or allergy testing, rarely really helps…the allergies are often worse in spring and fall, and that coincides with the worst season for most migraine sufferers, but it is largely from other factors(weather changes, our brain chemistry is different, etc.)….bad allergies can produce a stuffy nose and slight or mild frontal headache, but rarely much worse…the allergy meds do help many people’s headaches, at least a bit, but because they affect histamine, etc., not a direct anti-allergy effect. The food sensitivities that people with migraine suffer from are a chemical sensitivity to certain substances, such as tyramine, not an allergy. In my experience, with thousands of headache patients, treating the allergies helps the allergies, but only rarely the headaches. these are just my 2 cents, and I know that there are many other legit views on the subject. My problem has been that if I get a headache for ANY reason, and keep it awhile, it often triggers a migraine later. While I sat in the allergy clinic, getting injected with the various substances, a lot of them gave me an allergy headache. After keeping a dull allergy headache for most of the day, complete with both runny nose AND stuffed up sinuses, I went home and got a full blown migraine, complete with visual aura, nausea, vomiting, the works…That is my allergy/migraine connection, not so much that allergies CAUSE migraine. Well, after some sound and good advice from members here, I have decided that it is time to try and find a new pain management doctor. As some of you know, I have been dealing with the same doctor for a little over a year. I have grown more and more frustrated each month. After going in about two weeks ago, he decreased my meds after I told him I was not getting relief. I was very upset. I had a very bad weekend, and called his office today. Not only will they not "up" my dosage, they will not see me until late next week! I told the nurse that I need something to help. "Sorry, as you know, we do not change or prescribe new medication over the phone. Only after an office visit". I told her that I do not want to come in unless I can know for certain that a change of meds will take place. She could not do that. I asked about going to the ER – would he see me then? No, but if I request, he may take a phone call. ARGHHHH!!! Here I am, crying, literally begging for something to be done. She finally told me that he is out of "this" office until next week, but she will "try" and talk to him about me if and when he calls. Again though, I was told that no meds would be prescribed or changed unless I am seen. Also, non pain related, I am still suffering from this upper respiratory thing and have been since the last week of December. Coughing, low grade fever, generally crummy feeling. My PCP has treated me with 5 rounds of antibiotics, one antibiotic injection along with a steroid injection (in case it was an allergy or something), TB test, chest x-ray, sinus x-ray, an ER visit since I was having shortness of breath and pressure of my chest, and numerous rounds of prescription cough syrup and OTC "cold" meds. Only thing that had turned up was sinusitis (well, duh! who doesn’t have sinusitis in Spring!). I could never convince my PCP to do any blood work. I had my yearly with my ob/gyn in mid April. I told him what was going on. He did do a CBC, but it showed nothing except slight anemia. So here I am. Still running low grade fever. Coughing up junk. Feeling crummy. Back hurts worse than ever. My pain management Dr won’t change or up meds, and won’t do "cottle" (sp) injections again until I am over whatever is making me sick (by the way, first round of cottle injections did nothing but make me hurt more and even though I have repeatedly told him that, he still want to "try one more time"). My PCP Dr won’t treat me for this crud anymore and wants me to go to an ENT of which I can’t get in for over 4 weeks. I told my husband that I want to be admitted to the hospital and have every and any test done to find out what is making me cough and run a fever. While I am there, I want more tests done on my back – another MRI and anything else that might help solve my pain problem. So far, the only test that has been done for my back was an MRI done about 2 years ago that showed a herniated disk. I know there has to be something else. Pain is just too intense – low back with sharp pain from hip to little toe running down each leg. I can’t bend over, lift, turn, etc without feeling like a knife is jamming into me. I want relief. Thanks everybody, this is SUCH a nuisance! Now on top of all of this, I can’t take the Bactrim! It gives me horrible pains in the mid-sternum area; probably gastric. Going back to the doctor on Thursday and probably schedule for CT scan. Isn’t this a crazy mess…I NEVER had a sinus problem in my LIFE until now! There isn’t a day that I don’t get some kind of pain..in the head, face, etc. Guess there’s a first time for everything! Thank you tons for your replies! I have had the exact same thing for the past 3 yrs. I was originally diagnosed with sinusitis & given antibiotics. Nothing
has worked! I started getting that "bruised" scalp feeling & now
am losing hair! I can’t even pin my hair up anymore because the pain is intolerable. All this from sinusitis? Could sinus infection leave a person open to fungal infection? I hope

I have recently been told by my Doc that I have a sinus infection, and he put me on Bactrim. Just started it. The pain that I get along with the pressure around the eyes and nose, is intermittent "head pains", they don’t really turn into a full blown headache, and they come and go. ALOT! Also, it feels like my HAIR hurts,,scalp area I assume!! Is this normal with a sinus infection? Also, I have minor stuffiness…not anything unbearable. But have "blood traced gunk" come out about every morning. (sorry so grossly graphic!") Well this is the real News Flash 20 and its been a wet morning here. I left at the regular time ( about 1am ) and walked to the seat down the back ally and over the little bridge. I past the church and the public house and walked back to the shops. I did experience a little anxiety on the first 1/3 of the walk today but i thought it could have been the stress from last night and the past week so i ignored it. I carried on to the long path which is a fair way from the shops and walked right to the end and walked onto the new bridge which is the furthest i’ve been so far and then back up the same way. I past the same chap who was walking his dog again and i hazarded a smile in between the heavy breathing as the path is on a hill and at this point i was 2/3 through the walk but he didn’t respond so i think its best i avoid that the next time. I walked past the petrol station today and this means i’m going even further now so that’s a few more steps i’ve done
. I headed back towards my friends house as he said he would sellotape a letter to the front of his door which to my surprise he did ( insert a gasp here….reasons for the gasp…..1 he said he would come round to see me thursday and he didnt…..2 then i was told he would pop round saturday which he didn’t do either…….he did call round sunday but i was asleep as i had a rough night ). I un-stuck the letter off the door and started to read it on the last part of my walk ( narrowly missing his dogs who started to wake up as i left ). I was asked how i enjoyed the birthday and did i like the card and prezzie ( which i did
). I was also told that he would call here ( well i think its him that is calling but we may find its his lovely wife who calls so either way someone will call round, but i’m not holding my breath ok LOL ) on wednesday and also a friend of ours has just had another beautiful little girl yesterday which means i must try to expand my walk so i can post a message through their door as its close to his house. The baby was just over 3 lb and called Harmony Jean ( then the surname here ) which going on the names of his other girls ( Willow and Amber ) fits in very well. Anyway after i finished the letter i was nearly at the end of my walk as i could only read it while i was under a street lamp. I finished the last part and did the whole walk in 1 hour 28 minutes and 14 seconds ( and if you need more accuracy 54 hundredths of a second ) so i beat one speed record for me but stayed out a little less. Well its time i posted this now and made my web page up even tho i didn’t get any pictures as the rain was pouring down again today. (((Many Gentle Squishes))) to you all

Well this is the real News Flash 20 and its been a wet morning here. I left at the regular time ( about 1am ) and walked to the seat down the back ally and over the little bridge. I past the church and the public house and walked back to the shops. I did experience a little anxiety on the first 1/3 of the walk today but i thought it could have been the stress from last night and the past week so i ignored it. I carried on to the long path which is a fair way from the shops and walked right to the end and walked onto the new bridge which is the furthest i’ve been so far and then back up the same way. I past the same chap who was walking his dog again and i hazarded a smile in between the heavy breathing as the path is on a hill and at this point i was 2/3 through the walk but he didn’t respond so i think its best i avoid that the next time. I walked past the petrol station today and this means i’m going even further now so that’s a few more steps i’ve done
. I headed back towards my friends house as he said he would sellotape a letter to the front of his door which to my surprise he did ( insert a gasp here….reasons for the gasp…..1 he said he would come round to see me thursday and he didnt…..2 then i was told he would pop round saturday which he didn’t do either…….he did call round sunday but i was asleep as i had a rough night ). I un-stuck the letter off the door and started to read it on the last part of my walk ( narrowly missing his dogs who started to wake up as i left ). I was asked how i enjoyed the birthday and did i like the card and prezzie which i did
). I was also told that he would call here ( well i think its him that is calling but we may find its his lovely wife who calls so either way someone will call round, but i’m not holding my breath ok LOL ) on wednesday and also a friend of ours has just had another beautiful little girl yesterday which means i must try to expand my walk so i can post a message through their door as its close to his house. The baby was just over 3 lb and called Harmony Jean then the surname here ) which going on the names of his other girls ( Willow and Amber ) fits in very well. Anyway after i finished the letter i was nearly at the end of my walk as i could only read it while i was under a street lamp. I finished the last part and did the whole walk in 1 hour 28 minutes and 14 seconds ( and if you need more accuracy 54 hundredths of a second ) so i beat one speed record for me but stayed out a little less. Well its time i posted this now and made my web page up even tho i didn’t get any pictures as the rain was pouring down again today. (((Many Gentle Squishes))) to you all

I have intense pressure in all the sinus regions and likely have sinusitis. I just started taking antibiotics and using the Grossan irrigator. I don’t have much of a runny nose except when I go out for a walk in the cold wintery air, then it runs a lot, more than anyone I have talked with has experienced. Indicates the sinuses are being chilled & irritated by the cold wintry air – happens to me too. This is one reason why I live in California! Cover your nose with a scarf. Or wear a ski mask. When you get home after being out in the cold air, inhale some steam and then irrigate with warm salt water. Keep irrigating! I have come across a new blood test for food intolerances, allergies etc. that can be the cause of numerous health problems inc. sinusitis, fatigue etc. They screen for hundreds of potential foods, molds/fungi’s inc antibiotics, additives, colourings, etc. Information is available at www.alcat.com I am considering having this test myself and would be interested if anyone elso knows about it. As you know, I am in a different ballgame than you other Enbrel users. I have been in the clinical trials for more than three years now. I have never stopped the Enbrel because of an infection, (I haven’t had a cold in years) but you do what your doctor tells you to do. In the study, we just keep taking the Enbrel and I have taken it right along with sinusitis, bronchitis, and a bladder infection. I have gotten along just fine this way. But I have no underlying disease like diabetes either. I have taken various antibiotics along with Enbrel with no drug interaction or adverse effects. Also, I am not on MTX, only Enbrel. The longer I am on Enbrel and off all the other drugs like prednisone, the healthier I have become. Now I rarely get sick. IMO, drugs like prednisone lowered my immune system and made me very suseptible to URIs. You asked for experiences, and that is mine, but do what your doctor tells you to do.
Thanks, interesting post. I am also on a high dose of mtx so I have no arguments with being conservative. I am in full agreement with your statements about staying off drugs like prednisone. Enbrel has been a huge help. — MZ

Not yet Mama Char. I’m waiting till after Christmas and also I get my next enbrel batch on Dec. 29th. Have to do my clinical trial bit every four months now, and I get a four months supply of Enbrel. I stayed out of work today because I came down with a cold. My RD said not to take mtx and enbrel when I am sick so… no Enbrel or MTX for me today. Hopefully I will recover enough this week to take Enbrel on Thursday. I was wondering, how long off the Enbrel before I get a flare? I suspect this varies by individual, but I wondered if you Enbrel users had any experience with it. This will be the first Enbrel injection I have missed and it has been over a year since I missed an MTX dose. I am not looking forward to the rest of the week. Any experiences? — MZ

Hi Mary, I just started Enbrel, so I don’t have experience to share with you, but my RD said to stop Enbrel if I have a serious infection. When I asked how serious is serious, she said one that puts you in the hospital on IV antibiotics. She said I could call down anytime I have a question, but I took it that meant I wouldn’t have to stop it for colds or minor infections. My pain increased with the second Enbrel withdrawl. I had a few bad days but comapred to how I felt one year ago it wasn’t awful. I saw my doctor last week and he cut my MTX injections by half and wants me to try once again to taper my prednisone. (I’m on 7 mgs per day now) Thinks maybe the MTX is contributing to the frequent infections. So…your mileage may vary but I did ok for the 2 times I had to go off the Enbrel…but then again I am not working and could baby myself. Hope you feel better real soon and get back on the Enbrel. KAT

Thanks! We tried to reduce my mtx level but that ended in a flare. How quickly did you reduce your mtx level? I tried 3mg reduction and it was a disaster. I’d like to get it down but no luck. — MZ

Mary, I had to go off Enbrel for 3 weeks due to a sinus infection also. The worst symptom I noticed was an increase in fatigue. Towards the end of the three weeks, my hands started swelling really bad. Even through two weeks of Enbrel shots, they didn’t go back down to the size they were when I first started the Enbrel. So last week I got a 160 mg shot of DeproMedrol. They’re better now, but now my time is up for using enbrel that was set by my insurance carrier. I have one more box of shots and my doctor is calling them back to get another 2 months; or whatever they’ll approve. I will be so mad if they reject me. Donna F.

Remember here we are speaking of excess or heavy nose blowing. I have been preaching this for years – MUST blow nose very gently. This is why the various methods of irrigation that don’t control the pressure can spread infection whereas when the pressure is controlled – just so (just about one inch above the Water Pik stream from the Grossan attachment) it is beneficial. Right. Your reaction to the flu shot sounds intense (and unusual), but as you said, feeling like shit for 2 or 3 days is better than getting the flu, a fever, and being down for two-three weeks and then maybe having an exaccerbation or perhaps pneumonia. And the shot does NOT give you the flu, despite fokelore. Kathi

It was after having my first and only flu shot that I had the worst exacerbation — mildly ill for about a month after the shot, just didn’t feel quite right, and then a slide, then a big crash which lasted for months. I’ve been on Copaxone for almost 10 months and have not ‘caught’ any bugs. I also had a flu shot and noticed no problems with it. Guess it’s different for everyone like everything. Micky B/Memphis

Yes, I am on Copaxone also. I have not been sick. Everyone around me has had colds and flus. I have remained healthy…for me that is a miracle. I’ve been on almost 5 months now and while I’ve caught a few colds (I work in a school plus I have a small child, so germs are a given!), I haven’t had to run to the dr for antibiotics every single time like I used to. Every cold used to either become a sinus infection or bronchitis, no matter what self-care measures I took. Now, rest and fluids do the trick. Tammy — I’ve been taking Copaxone for two years, and have noticed that I’m more likely to catch the latest bug. The plus, though, is that when I used to get a cold, I would promptly relapse. Now I just enjoy the cold, and then move on with my life, fingers crossed. Last fall I got chicken pox (for the third time in my life — I don’t know WHAT that’s about!) and held my breath for the ax to fall. It didn’t fall. Still, you wouldn’t catch me in line for a flu shot. In the meantime…a bus full of kids? Like, every day? Egads wears me out just thinking about it! The only thing that seemed to help at all was to dash into the restroom every chance I got, and wash my DRY hands! I could’ve kept Jergen’s lotion in business!)
