Question:
Hi, I got what I thought was a heavy cold a couple of weeks ago, and a few days into it my nasal discharges got pretty bad and the nasal blockage didn’t let up. Went to the docs and got a 7-day course of amoxocillin which I have just come to the end of. Things now seem to be getting worse; the discharges are thick green (previously clear and white), the blockages are still there in both nostrils alternating between left and right nostril but not both at the same time and much worse on the left side. When I blow hard I am getting what also seems like ‘blocked ears’ ? and last night got a pain which felt like it was behind my left eye. I have tried homeopathic nasal sprays, irrigation with warm salt water, Sudafed tablets and steam inhalation with menthol crystals. None seem to work; although steam inhalation gives me about 5-minutes relief where my sinuses seem to clear, but the menthol made my eyes water really badly. Can anyone tell me what the Dickens is going on? My missus also seems to be getting wound up with my pre-occupation of trying to get well. Regards TM P.S. Just remembered that a couple of days before I got sick, I had an irritating painful pustule in my left nostril which I zapped. Could this be a contributory factor?

Perhaps some of the bacteria from that painful pustule in your left nostril, which you zapped, was able to spread into your nasal areas and turbinates? After I suffered with multiple sinus infections, year after year, for decades, I experimented with flooding my nasal passages with a mixture of diluted 3% hydrogen peroxide, baking soda and kosher salt. In order to flood my nasal passages and keep them flooded, I keep my forehead pointed downward. I keep my forehead down for quite a while, hoping that the anti-infective mixture can get to the infected area(s) within my sinuses and nasal passages. . It works! I have been sinus infection free for about 3 years. I think that I had chronic infection problems with my turbinates, so flooding my nasal passages with the anti-infective mixture worked very well, for me, since the infected areas were fairly easy to reach. . I have fully documented my experience and how I did it at Healthboards.com. There are posts there by people who have tried my "upside down sinus flooding" and have received either partial relief or total relief. There are also posts there from those who it hasn’t helped, and by those who don’t think that it is a good idea. I have no connection at all with Healthboards. Hi thanks for the info and links. There’s loads of positive info there. Some questions…, I have never heard of kosher salt., what is it and where can I get some? Also is hydrogen peroxide that stuff that people use for bleaching their hair? Presumably you would buy this from the chemist? One last sympton, sorry to be so graphic, but I since I have been ill with this sinus trouble, I have also had a sore throat, although this is off and on – when I clear mucus from the back of my throat, I am also bringing up small hardened green/brown lumps that smell disgusting; it feels like bits of peanuts. My missus reckons it part of my lungs. Have you ever heard anything like this and is it related?

65% is about par for the course. The only scientific followup study on FESS, done by the Henry Ford Hospital in Detroit, showed that the cure rate was around 75%. So with a more difficult case, 65% isn’t at all out of the ordinary. FESS is not a guaranteed cure for chronic sinusitis. — Steven D. First of all, there is nothing wrong with taking a steroidal sinus spray indefinitely. That is ok. (Oral steroidss , on the other hand, are a different situation). I agree with Steven that you need some more opinions, preferably from a major University medical center. Do not think for a moment that, in a difficult case, all ENT’s are equal. The number 65% may be his way of saying that he is not sure at all of success. I would want better odds if I could get them.

Hi, Last winter I got a very bad case of pneumonia, which was followed by sinusitis. Unfortunately the sinusitis wasn’t diagnosed until it had become chronic sinusitis. I was treated for extended periods of time with various antibiotics, which didn’t help at all. In addition to "regular" sinus symptoms (headache, green discharge from the nose, etc) I continued to have a bad cough, shortness of breath at times, and my GP could hear noise in my lungs. It was a pulmonologist who said that if I clear up the sinusitis it will resolve the cough and problems with my lungs. At the beginning of the summer I began prednisone, which did finally calm things down. In September I stopped the prednisone and remained only with nasal steroid spray. That has things under control, but I am not able to stop it without all the symptoms returning. My ENT says that I definitely "deserve" a FESS operation. All of my sinuses are blocked except for the frontals. However, I haven’t done it yet because the ENT says there is only a 65% success rate in situations like mine. On the other hand, I don’t like the idea of using the steroid nasal spray endlessly. What do you think? Does anyone have any advice for me? I would really appreciate it.

I think you should get a second opinion from another ENT. Especially since your case may be a more difficult one. Does this med not have the traditional problem associated with NSAIDs like aspirin or ibuprofen, internal bleeding? And of course we now see a major problem with one of the "new" prescription NSAIDs, VIOXX. Is this med at all similar to VIOXX? Or—completely different from both??

No, i guess VIOXX was a COX-2? inhibitor, Mobic is in some other group that has to do with ‘enolic acid’. Doc told me Mobic is one of the newer kids on the block and far as anti-inflams go. It’s most often prescribed for osteoarthritis.

Could it be the same as when using Prednisone it compromises your immune system thereby allowing other bacteria/virus to attack your lungs? As with so many meds you have to weigh the benefits versus cost factor for your health. If you are in a weakened condition already it is scary to be put on meds that exacerbate that. Trudy.

thank god for the internet. i just talked to my primary care physician and again mentioned my continuing problems with inflammation in my sinuses. he prescribed an anti-inflammatory med for me called Mobic – which I guess is a relatively new class of these type meds. Well, I just went to check on the possible side effects and upper respiratory infection is one of them. then, i found a few posts on discussion groups by people who’d suffered this side effect. i already took my first tablet of the stuff. that will certainly be my last. i can’t afford an upper respiratory tract infection. i’m wondering if anyone knows how/why this anti-inflammatory could cause or contribute to developing upper respiratory tract infections? iJah

iJah, a search of the National Library of Medicine Medline database found no listings of studies or papers on the subjects "mobic AND sinusitis". I’m not sure why your physician would try a medication intended to relieve pain from osteoarthritis. Perhaps this is some "off-label" use that has been found effective? — Bruce

thanks for looking Bruce. it turns out that Mobic is often prescribed for asthmatics because it is a ‘leukotriene’ antagonist. And, I just read on a site regarding treating inflammation in chronic sinusitis with steroids that: (Other anti-inflammatory agents called leukotriene receptor antagonists and are showing promise in treating this condition) So, I guess my doc figured this Mobic held some promise. Maybe it really does. But, I have seen listed as I mentioned, on a number of web sites and discussion groups that ‘upper respiratory tract infections’ is a side effect. Usually bronchitis – i did not see any mention of sinusitis as a side effect. I can’t afford a bought with bronchitis in the weakened frail state i’m in now, so i figure i have to avoid taking mobic because of the risk of that side effect. iJah

Don’t worry about it. I don’t only have sinusitis, I also have a kind of chronic bronchitis caused by years and years of sinus infections. Hello, I am considering FESS to open up my ostia to reduce the worst symptoms of my chronic sinusitis. I realize it won’t cure the underlying causes (which I deal with in other ways), but hopefully opening up the ostia will improve drainage and bring more oxygen into the sinuses (thus reducing the anaerobic bacteria blooms which sometimes causes really putrid, room-filling halitosis when all of a sudden I get a lot of drainage for whatever reason.) I’d like others who have had the surgery to share their experiences. Also, I’d like to hear from ENTs and MDs reading this to share their general views (I realize you can’t make any recommendations specific to my case, but your general comments will be appeciated by many of us.) Btw, what "natural" remedies help with reducing inflammation around the ostia and to open them up? Thanks! Mark (please reply here since your feedback will benefit others.)

Of course, you realize that every case is different. I had a lot more surgery done to me than just opening up the ostia. So whether the FESS will help, what kinds of risks are involved, etc., is highly dependent on the individual. This NG has been around for quite a while, and over the years, lots of people have posted their experiences with FESS. I suggest you use the Google archive of this NG to search past posts on this topic. Based on my own experience and those of others, however, I would strongly recommend that you authorize your surgeon to identify and, if necessary, repair whatever other problems he finds in the course of the surgery. I have had two surgeries so far, and each time, my surgeon found other problems inside my sinuses that he hadn’t known were there–and so it’s a good thing I gave him pre-authorization to fix whatever he found needed to be fixed. — Steven D. I think people do take them together but Claritin, an antihistamine, is a drying agent. Is that what you are trying to accomplish? Dry out watery secrtions? A lot of ENT’s do not like like antihistamines because they can inhibit full draining of the sinuses. They may be contraindicated in chronic sinusitis.

My MD prescribed the flonase and clarinex for a week for my upper respiratory infection. The clarinex at night as it makes you tired during the day. I asked about taking Claritin and she said the clarinex, it works better. I take Claritin for my Hay Fever which it is very high now. It is a melting one that melts in your mouth. Hello all I have been struggling with sleep apnea for about 5 years now, although I only knew I had it about 2 years ago after a sleep study. After the sleep study I was told I had moderate OSA. I stop breathing about once every 3 to 4 minutes. My doctor prescribed a CPAP machine for me and set it at Min 4 Max 10 setting. So fortunately my insurance paid 100% of my cpap machine, a ResMed autopap, I forget the model number at the moment, but it came with a humidifier. So I tried this and never sleep through one night as it was pretty claustrophobic for me and I was not able to breathe well through my nose as one side of my nose was blocked. I tried the mask that came with the unit, I tried the nasilAir 1 & 2 and tried nasal pillows , they all failed horribly for me. This was during a 10 to 12 month period. I then went back to my allergist to see if I had any issues with allergies that might cause my nose/nasal are to be blocked… he sent me to a ENT and they took a MRI of my head and found I had a very bad case of sinusitis(chronic) so I went on antibiotics for 3 weeks and then went back for x-rays. There was still some remaining "STUFF" in my nasal cavity, so they said we will operate to remove the rest and at the same time perform a turbinate reduction to help my breathing with the CPAP machine. So very frustrated I went back to my sleep apnea doctor here in Montreal and asked him I am desperate what can I do? he said call up this oral surgeon and make an appointment for an Oral appliance fitting. So I went and saw this doctor and she said I was a good candidate for this appliance as I was not overweight, I had only moderate OSA, and my jaw structure was good. so I went for it, costing $1700 out of my pocket because insurance or Medicare here in Quebec does not pay for it. So the first night I used it my snoring and apneas went down by 80% my wife said… all went well for 5 nights until I got pain in my jaw on the right side, so I called the doctor, and she said to stop using it till the pain is gone. so I did, but the next morning the pain was worse, even though I did not use it that night, I could not even chew my food for 3 days. But after 3 weeks the pain went away. my doctor adjusted the oral appliance back 1mm, and I tried it again…… no pain this time but my snoring returned. my wife said there was no reduction at all… so now I am waiting for my doctor to come back form vacation so she can adjust forward 1 mm or more again. Although the doctor did mention the snoring might come back but after an adjustment it should be ok for a year or so….. So this is where I am at now. Having suffered sinusitis for 5 weeks after recently moving to South China I felt I must post info about my surprisingly simple cure. Large doses of Vitamin C are extremely effective on many viral and bacterial diseases and I found the information at doctoryourself.com/titration.html – I tried two different antibiotics with a five day gap between drugs to see if I could kick the problem with my own immune system but no effect at all. If you take between 3 an 6 grams of Vitamin C in tablet form a day you will find symptons will reduce within hours and after that recovery starts to increase. I am now on day three of recovery and feel 100per cent better – I still have some occasional mucous and a cough which I will do some more work on but I feel totally better. The explanation of how Vitamin C/ascorbic acid works is on the website I have mentioned – do not be afraid about taking large quantities there is no side effect if you are ill – if you do not need Vitamnin C your body simply does not absorb it and the worst that can happen is that you get the runs. I have not suffered from stomach upsets on 4/6 grams a day which my body is currently lapping up . I have reduced doses to 3 grams now on day 4 which seems to be about right now, I will keep reducing this as I finally clear the cough etc. Good Luck – It actually works!

This message is for Steve L if you are still out there… Steve, I just had my second operation last week and and recovering at home. I had one of the best ENT surgeons in China operate on me and feel pretty confidant that he did the best anyone else could do. He said that whoever did my first surgery did a very conservative job. In any case, although I was told that the surgery was sucessful I am still suffering from this damn ear popping. You said that you had the same problem before. I even went back and saw some of your postings from a couple of years ago on ear popping. My question is how long after your surgery did the ear popping go away? Thanks Steve. 1. Yes, I moved to a new apartment with much better ventilation after my infection last January. But, I still got infected again in April. I tried to hold off on taking anti-biotics for a week but it just got worse. I don’t think its related to my accommodation. I think 50% of the problem is living in Beijing. High pollution levels. 2. I use and love sinus irrigation. I just use warm distilled water and salt. 3. Problem is totaly exaceberated by living here but it would be really difficult for me to leave a job and life I love here so much. I am considering it though. 4. I’ve been to over a dozen GPs and ENTs. I’m starting to get confused. General message is, ya, it looks like you have a problem, no blatant obvious structural problems, but sure we should probably try surgery again now. Maybe its that concha bullsa or maybe your turbinates have gotten bigger again. Perhaps my experience might be instructive: My CT scan didn’t show blocked sinuses either. And so ENTs kept looking at the CT scan and concluding that I didn’t have sinusitis. I finally found an ENT who knew that even CT scans aren’t 100% reliable–there’s about a 5% chance of false negatives. sinusitis means swollen sinuses, thats all. the other ideas put forward are great imo. You can use colldial silver or traditional antibitics in your irrigator. After suffering with multiple sinus infections, year after year, for decades, I experimented with flooding my nasal passages with a mixture of diluted 3% hydrogen peroxide, baking soda and kosher salt. I kept my forehead down for quite a while, hoping that the anti-infective mixture could get to the infected area(s) within my sinuses and nasal passages. . It worked! I have been sinus infection free for about 30 months. I think that my turbinates were infected, so flooding my nasal passages with the anti-infective mixture worked very well for me, since the infected areas were fairly easy to reach. . There are posts over there by people who have tried my "upside down sinus flooding" and have received some relief, those who it hasn’t helped, and by those who don’t think that it is a good idea. I’d appreciate any advice that you have for me. I get diagnosed with URTI or Sinusitis 5 – 6 times a year. Nearly every time it gets out of control and I end up on anti-biotics and am ill for 3 weeks or more.
