My surgery

Question:

My surgery is scheduled for June,16th Its awful in there! I knew the moment he put my films on the lightbox! I could clearly see the severe blockage on my Left side! Im thinking of calling a lawyer since I never, in the 5 yrs Ive been suffering, have been referred to an ENT by my regular doctor! It was like a slaughterhouse doctor mentality! Push em in treat em script em and leave! Never so much as a mention! Not only that but I’d complain of fatigue and they’d blame it on depression and script me SSRIs! This is the SAME doctor that told me I didnt have rosacea… VOILA! A dermatologist diagnosed me with it! I don’t have an infection, but I have that post nasal drip sore throat and swollen glands! At least I know now and Im happy… Im gonna scan my films and post the pics if I can! If you can type in the radiologist’s report, that will be very useful too. That will give the radiologist’s analysis of the scan.

You mean your sinus problems?  Absolutely!  With me, sudden loss of sense of smell is often the first sign that my sinuses are starting to block up; that has led to infection (though not always). Surgery carries with it a small risk of loss of sense of smell too.  If your surgeon hasn’t already discussed all the risks with you, ask. You’re supposed to sign a form stating that you know the risks and are prepared to take them. But in another post you said that your surgeon is only going to do a septoplasty and remove some polyps.  I think you should get a second opinion.  You want a surgical procedure that is neither too conservative nor too aggressive.  You don’t want to have to go back for a 2nd surgery if you can avoid it. — Steven D. Surgery was performed on March 24.  The ENT removed multiple polyps in both nostrils, corrected an irregularly deviated septum and performed work in my ethmoid sinuses. I am feeling great now. Could this cause me to lose my sense of smell?

Whoops, I should have looked over here before I posted over there. The best advise your getting here is a second opinion and make sure it’s with a full endoscopic exam. Make them tell you what they plan to do ahead of time (ref. my other post to you). I still think the Gentamycin irrigation is worth a shot from now until your surgery, just to see what happens, sinus problems are tricky some CTs show horrible images but the patient has no symptoms? Don’t be disillusioned that ESS is going to cure post nasal drip, allergies, or prevent you from other sinus care like irrigation because it won’t, that even the ENT will tell you. Have you considered the Mayo clinic’s fungus theory and irrigating with an antifungal mix? I hope you get your life back soon, intact.

I had a similar experience with my primary physician.  Every time I saw him about completely stopped up conditions due to colds he NEVER EVEN LOOKED UP MY NOSE. Instead he prescribed Bactrim, Cipro or some other antibiotic which sometimes worked and sometimes didn’t.  Only after I saw an allergist did I know that I had a serious case of multiple polyps with a possible call for surgery.  An ENT specialist examined me and in addition to the polyps, I also had an irregularly deviated septum and severe congestion of the Ethmoids. On his recommendation I did have surgery. Now hopefully conditions will be righted. This whole situation infuriates me.

Agreed – people should NOT go to surgery until they get some second opinions, preferably including one from a conservative source (e.g Mayo).

I cleaned up the image a bit to enhance contrast–I’ll email it to you. Yes, you’ve got thickened lining and maybe polyps in your left maxillary sinus (at least). But your right side isn’t so great either.  It looks like you’ve got some blockages in your right ethmoid sinuses, and some thickened lining in your right maxillary sinus.  The thickened lining in your right maxillary could cause problems because it extends all the way up to the top of the sinus, where the duct (ostium) is that drains the sinus.  If the duct is blocked, then the sinus can’t drain and infection can easily set in. So my guess is that you’re headed for a bilateral antrostomy and maybe an ethmoidectomy. I know how you feel.  I was misdiagnosed for 4 years too, and in that time, everything got worse, of course. But medical malpractice is very hard to prove.  You would have to establish that your physician didn’t follow established medical procedure.  Given that chronic sinusitis is hard to diagnose, and that good diagnosis is still a judgment call sometimes, I doubt you could win. > Push em in treat em script em and leave! Actually it gets worse. Antidepressants can have a drying effect on the mucous membranes and might have made your sinus infections even worse. > I don’t have an infection, but I have that post nasal drip sore throat and > swollen glands!

How did your ENT decide you don’t have an infection?  Swollen glands are usually a symptom of infection. Don’t forget to get a second opinion before having the surgery!  In my case, the ENT whom I consulted for my second opinion impressed me so much that I decided to have him do the surgery instead. — Steven D. Well, I just briefly perused your website and saw you live in "Philadelphia" area.  There is no excuse for the poor treatment you receive from your PCP or the slow reading of your films.  Philadelphia is actually a great place for medical treatment. Sheez, from your posts, I thought you lived in Canada or England with government "health" insurance. You have a choice – take it.
